Forms should be completed prior to your child's appointment with Children's Physician Group–Orthopaedics and Sports Medicine as this will help reduce wait times.
Location updates
- Buford: Children's has decided to permanently close our orthopedic and sports medicine clinic in Buford. This change is part of a thoughtful consolidation of services, allowing us to meet patient demand within our community with the appropriate number of locations. Families will have the opportunity to transition care to one of our nearby locations. The last day of clinic will be Friday, Dec. 17, 2021.
- Fayette: Children's will be moving its specialty services, including orthopedics and sports medicine, from Piedmont Fayette Hospital to a medical building across the street. The new address will be 1250 Highway 54 West, Suite 200. The new location will provider a larger, more modern space along with easier accessibility for our families. The last day Children's will hold clinic inside Piedmont Fayette Hospital will be Wednesday, Jan. 5, 2022. Clinics will resume in the new building on Monday, Jan. 10, 2022.
